Job Title: Registered Nurse (RN) – Home Health
Location: 226 Northpark Dr., Rock Hill, SC 29730
Coverage Area: York and Lancaster Counties (SC); occasional float to Union or Mecklenburg County (rare)
Employment Type: Travel Contract
Position Overview:
We are seeking an experienced Home Health RN to join our team in York County, SC. This role involves providing high-quality, compassionate care to adult patients in their homes, with a focus on chronic disease management, wound care, and patient education.
Workload & Scheduling:
Daily Caseload: Minimum of 8 patients/day, with a requirement to meet 9 visit points per day
Shift Times:
Weekdays: 8-hour shifts (08:30–18:00), flexible as long as 8 points are met
Weekend-Only Roles: 12-hour shifts (08:30–20:30), must meet 9 points/day
Weekend Rotation: None, unless hired for a weekend-only position (Fri/Sat/Sun)
Call Requirements: None
Holiday Coverage: Required as scheduled
RTO: Subject to manager approval
Orientation: 1-week fast-track onboarding
Experience & Qualifications:
Minimum Experience: At least 1 year of Home Health nursing
First-Time Travelers: Not accepted
Licensure & Certifications:
Active RN license
BLS certification
Valid Driver’s License and proof of auto insurance
Patient Population & Common Diagnoses:
Adults only
Chronic medication management, cardiac conditions, heart failure, COPD, wounds, and drains
Required Skills:
Proficiency with Homecare Homebase (HCHB) – mandatory
Experience with OASIS-E assessment tool
Strong clinical skills in:
Patient assessments
Wound care and wound vacs
Pleurx drain management
Infusions and venipuncture
Start of Care (SOC) visits
Chronic disease management and medication teaching
Case management and plan of care development
Preferred: Experience with CADD IV pumps
Technology & Equipment:
EMR: Homecare Homebase (HCHB)
OASIS-E for assessments
